Kei is something of a special case within Kamui's limited group of acquaintances. He gets this honorable privilege because of his backalley clinic. No matter what Kamui's personal opinion happens to be at the time, Kei is someone he has to keep around if he wants to maintain his rough lifestyle, and this is not something Kamui is used to. At all. Compromise, vulnerability, and empathy are all traits he's out of practice with, even in their most basic forms.
He's learning the hard way what complications this entails. Especially since the dickhead is so damned observant. Sometimes this pseudo-intimacy is hilarious, and then sometimes Kei hits on a weakness that Kamui is absolutely not allowed to have, and the world comes crashing down around him. Yet the next time Kamui gets punched in the face, he still has to be able to go say hi to the guy. Kamui won't ever run away from this situation because anything that will make him stronger is something he'll gladly accept. The problem lies in his definition of strength and his inability to reconcile it with actually caring about other people.
What this whole big mess is leading to is that Kamui is actually starting to care about Kei's opinions, and he hates that he cares, so he desperately tries to prove he doesn't care because caring is weak, and he's not weak. Except that he knows he's weak, and he knows Kei is realizing he's weak, and that's totally fucked.
