Rather than from smoking, his bad lungs are more of a physical problem, burning out quickly and even though he can ignore the pain, he's definitely the slower of the two of them.
Nothing really stops them on their way to the first checkpoint, but by the time it's in sight, Minato's definitely being showered with colored dots, each time he gets tagged making him an even more obvious beacon of somebody who sucks so much at this game, he doesn't even have a gun to fight back with. ]
